Scopo del lavoro
Valutare la funzione retinica e la stabilità della fissazione nel disturbo depressivo maggiore (DDM) e studiare l’influenza della terapia antipsicotica sulla qualità della fissazione utilizzando il microperimetro MP-1 (Nidek Technologies).
Metodi
Sono stati arruolati 28 pazienti con DDM (di anni 62,5±12,5 ) con acuità logMAR ≤ 0.04 e 30 soggetti sani (SS)(63.62± 14.14 anni) con acuità logMAR ≤ 0.0. Sono stati inclusi episodi singoli (296,2) e ricorrenti (296,3) di DDM in conformità con la IV edizione del Manuale diagnostico dei disturbi mentali (DSM IV). Sono stati esclusi quei pazienti con altre patologie mentali o con altri disturbi oculari diagnosticati. Sia dai pazienti con DDM che dai SS sono state acquisite la sensibilità retinica, la stabilità della fissazione, i punti di fissazione entro i 2 ed i 4 gradi e la bivariate contour ellipse area (BCEA). Tutti i pazienti affetti da DDM erano in terapia con inibitori selettivi della ricaptazione della serotonina (SSRI). Inoltre 12 pazienti assumevano anche antipsicotici atipici (gruppo A), 16 pazienti solo gli SSRI (gruppo B). La BCEA media (deg²) è stata normalizzata con una trasformazione logaritmica (Shapiro-Wilk test, p<0.05) ed è stata considerata la prima deviazione standard (68,2%). L’analisi statistica è stata effettuata utilizzando ANOVA con il test di Scheffe e la correlazione di Pearson.

Introduction: Age-related macular degeneration (AMD) is the main cause of visual impairment in the developed world 10 . The onset of the disease is characterized by damage to the outer retinal cells and retinal pigment epithelium. These processes can elicit a cascade of inflammatory and angiogenic responses leading to the growth of abnormal new blood vessels (neovascularisation) underlying the retina which can cause severe and rapid vision loss due to hemorrhage and exudation 1,2 . This form of AMD is called wet AMD. Exudative or wet AMD is a late form of AMD (as distinguished from atrophic, so-called dry, AMD) and is responsible for 60–80% of all cases of blindness due to AMD. As soon as neo- vascularisation becomes established, several pro-angiogenic mediators begin to play a pivotal role. Vascular endothelial growth factors (VEGFs) are the most specific and potent regulators of angiogenesis 2 . The introduction of anti-VEGF therapy caused a ‘Copernican revolution’ in ophthalmology. Intravitreal administration of these agents, which consists of injecting anti-angiogenic agents directly into the vitreous humor, has been used since 2005 worldwide for the treatment of neovascular AMD (n-AMD) and currently represents the primary therapy for this condition 1,3 . Three anti-VEGFs (pegaptanib, ranibizumab and bevacizumab) are currently used for the treatment of patients with n- AMD.

Purpose and materials: The aim of the present work is to report our experience in transplantation of limbal stem cells grafting in a 26 years old patient with a history of alkali burn in his left eye.

Method: After demonstrating of a limbal stem cells deficiency in the patient, a portion of tissue from his right eye sclera-corneal ring was collected and placed in culture medium for the multiplication of stem cells. It was subsequently carried out the grafting of the magnified tissue in his left eye. Patient was submitted to a follow-up at 6 and 10 months and then to the practice of penetrating keratoplasty.

Results: Six months after treatment the flap appeared transparent and the suture present and tight.

Conclusions: Limbal transplantation represents a new and efficient therapeutic solution. In the case of a severe limbal deficiency the only stem cells graft is not sufficient to restore the corneal transparency, so we recommend to perform subsequently a penetrating keratoplasty.

Introduction Impaired vision is a condition whereby a patient, affected by not reversible neurophthalmic pathology, is subject to a reduction of the visual function that cannot be corrected with standard lenses and results in visual disability in respect of the necessary visual needs (1, 2, 3, 4). In recent years the definition “low-vision” has certainly become more familiar in ophthalmology as this condition has become more frequent for two reasons: the senile population has increased steadily and as consequence the degenerative pathologies, which mainly affect the aged, are increasing; on the other side, the level of ophthalmic therapy and prevention has increased dramatically, especially after the 1970’s, this has reduced the number of patients whose illness resulted in blindness, and conversely increased the cases where therapy allows to maintain residual vision which if not useful in itself may at least be utilized (4). Patients may be defined low-vision for a variety of reasons. Some have impaired vision because of a central scotoma in their field of vision, others suffer of a general retinic sensitivity reduction, others are subject to tubular contraction, or various amputation of their visual field (3, 4).
